Medical Center of Delaware, Wilmington, Del. ‘“At-home bleaching” with 10% carbamide peroxide and vacuum-formed splints is gaining wide acceptance as a simple, safe, and effective technique to lighten teeth (Fig. 1). Among its advantagesover in-office bleachinglights and wands are (1) lesschair time for the patient and the dentist, (2) less operative and postoperative discomfort, (3) easeof “touch up” treatments if necessary,and (4) usually lessexpenseto the patient. A disadvantage of vacuum-formed splints used to hold the carbamide peroxide solution against the teeth is the inability to bleach a single darkened tooth (for example, a discolored endodontically treated tooth). A resolution to this problem is the useof a preformed plastic crown form to hold the solution against the tooth. BAssistant 10/4/31350

1. Select a properly sized crown form (L. D. Caulk Co., Milford, Del.). 2. Trim the crown form, leaving enough on the interproximai surfaces for retention and enough to cover the entire labial surface (Fig. 2). 3. Instruct the patient not to wear the crown form while sleepingor eating to prevent aspiration or swallowingof the crown form.
