Guideposts for a journey We have had two very productive days and accomplished a lot through your sharp and focused thinking about the future of ambulatory care pharmacy practice. The work at this summit will have an immense influence on shaping the patient care role of pharmacists in all ambulatory care settings. Although it is satisfying to look at the consensus recommendations we debated and refined, they are not an end in themselves. Those recommendations merely serve as guideposts for our journey toward ensuring that all ambulatory patients have timely access to the clinical expertise of pharmacists. ASHP and the ASHP Foundation know that the recommendations of the summit have to withstand scrutiny by the profession at-large, so we will seek feedback and buy in from a larger segment of the profession than could be assembled here in Dallas. After we have done that, we will publish the proceedings of the summit, which will allow us to share the recommendations and the background that supports them with the widest possible audience.

Research opportunities Many summit recommendations have implications for research, and the ASHP Foundation will give priority to supporting studies that assess the impact of ambulatory care pharmacists on patient outcomes. We will also support the development of patient care and business models that place ambulatory care pharmacists in direct patient care roles. Self-assessment A specific goal of the ASHP Foundation is to fund the creation of a self-assessment tool that practitioners can use to evaluate their capacity to provide progressive ambulatory care services. The tool will also provide a framework for advancing a practitioners’ care models consistent with the recommendations of the summit. State initiatives There is an old adage that says all politics is local. The same can be said of healthcare. The ASHP Foundation recognizes the value of state-based initiatives in expanding the vision for ambulatory care pharmacy practice and in establishing goals and priorities for achieving that vision. Extending a program that started with the

Pharmacy Practice Model Initiative, the Foundation will offer grants to state societies of health-system pharmacists for worthy ambulatory care initiatives. Building on insights from the summit Based on your experience at this conference and summit, I encourage you to build on your insights about advancing ambulatory care pharmacy practice. Suggest ideas for research to us. Share your thoughts on other projects that the ASHP Foundation might fund to drive change and improvements in the ambulatory care role of pharmacists. And please share your vision, knowledge, and enthusiasm for this topic with colleagues in your circle of influence. The time and effort you spent at this conference and summit will yield big dividends for the quality of the medication-related care of ambulatory patients and the advancement of our profession.
