Cardiac Dysrhythmias in Children

Before and After Balloon Aortic Valvuloplasty

Gillian A. Carpenter, MD, Stephen R. Shapiro, MD, John T. Cockerham, MD, and Stanley D. Beder, MD ecent reports documentthe successof balloon aortic R valvuloplasty (BAV) in children with congenitalaortic sten0sis.rConduction systemabnormalities including severebradycardia,2 prolongation of the corrected QT interval3 and ventricular arrhythmias,1*4have been reported during and after BAV. There havebeenno reports documenting the frequency of dysrhythmias both imme diately beforeand after BAV. This study assesses rhythm abnormalities by Holter monitor recorclmgsbefore and after BAV in children. The study group consistedof 12 patients with aortic stenosis who underwent BAV betweenMay 1987 and March 1990. Three of the 12 patients also had coarctation of the aorta, including 1 patient with Shone’scomplex. Patients ranged in age from 4 days to 15 years (mean6.4years). The meanpeak systolic left ventricular to aortic pressure gradient decreasedfrom 70 mm Hg (range 50 to 100) before to 31 mm Hg (range 4 to 57) after BAV (p
