CLINICAL STUDIES IN PATIENTS WITH CARDIAC DISORDERS A. COSTE-SIMONIN & D. KRANTZ H6pital Bichat, 1 70 Boulevard Ney, 75015 Paris, France

Two studies have been described previously (CosteSimonin & Krantz, 1975). The first was an open study in 30 patients with cardiac disorders receiving oral anticoagulants, who had accompanying anxiety symptoms. This included patients with coronary heart disease (21), valvular heart disease associated with arrhythmias (6) and other patients with cardiac arrhythmia (3). Patients were treated with clobazam 20 mg or 30 mg daily for 3 months. Apart from the anxiety symptoms, particular attention was paid to effects on blood pressure and to possible interaction of clobazam with the anticoagulant treatment. Prothrombin times were monitored for all patients for the 3 months preceding the study and during the 3 months of treatment. Clobazam was shown to produce no significant effect on prothrombin levels or on blood pressure. In all patients clobazam had a beneficial effect on symptoms of anxiety, which was also reflected in a reducReference

tion in cardiovascular symptoms. The second study was a multicentre double-blind comparison of clobazam, diazepam and placebo in patients with cardiac symptoms but no demonstrable organic heart disease. Six clinicians were involved and 135 patients were admitted to the study. Patients were randomly allocated to treatment with placebo, clobazam 10 mg, 15 mg or 20 mg daily, or diazepam 5mg, 7.5 mg or 10 mg daily. Patients were assessed after 2 and 4 weeks using a self-rating seven-point global scale. Comparison of the treatment groups revealed that the only significant difference from placebo was shown by the clobazam 20 mg group (P
