The left ventricle progressively dilates in some patients after acute myocardial infarction (AM). Both systolic and dlastolk left ventrkular (W) dysfunction can be of s@dfkance in the development of heart failure. Captepril has been shown to prevent dllatatlon, but the effect on LV dlastolk function is unknown. In a placebo-controlled deuMe-blind parallel study, 66 AMI patients with heart failure or low eject&n fraction, or both, were consecutively randomized at day 7 to either placebo or captopril(26 mg twke daily). No differences were present between the groups at baseline. Plfty-three patients completed the &month study period. Both LV dlastolk and systolk velume indexes increased signifkantly in the placebo group (17 and 14%, respectively); in the captopril group there was no change in l.V diastolic volume index, but a 13% reductlen In LV systolk volume index. Ejection fraction increased slgnifkantly in the captopril group. The peak flow velecities of the early and atrial filling phases were measured, and the ratio between the velecitier was cakubted.
