A 46-year-old woman with an history of colon cancer treated with left hemicolectomy, chemotherapy, radiotherapy and pulmonary nodule resections was referred for a series of fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT to assess the disease status. The last FDG PET/CT revealed a focal uptake to an endotracheal lesion almost completely obstructing the lumen (arrows); PET scan, endotracheal FDG uptake, maximum intensity projection (a), PET sagittal view (b), CT sagittal view (c), axial view (d). The patient underwent bronchoscopy which restored the patency of the tracheal lumen. Morphological assessment and immunohistochemistry identified the excised specimen as metastatic adenocarcinoma of the large intestine (CDX2ck20 positive, ck7-TTF1 negative). From 20 to 50 % of primary extrapulmonary solid malignancies show pulmonary metastases during their biological course [1]. Endotracheal/endobronchial metastases from nonlung cancer are rare and might be mainly associated with breast, colon and kidney adenocarcinoma [1, 2, 3]. The main clinical symptoms are cough, haemoptysis and dyspnoea and occasionally are an incidental finding during bronchoscopy [2].
