DISORDERS: A PRACTICAL GUIDE FOR

Schnyder and Cloitre state that their purpose for writing Evidence based treatments for traumarelated psychological disorders: A practical guide for clinicians was to create a text written by experienced researchers that was targeted for practicing clinicians—and they were very successful in this endeavor. The writing is very accessible; the authors did well discussing the

1

material in a very understandable way. The book is comprehensive and covers the essential information about posttraumatic stress disorder (PTSD) and other trauma-related disorders to assist clinicians in conceptualization and treatment planning. It is also internally consistent; the

t

chapters in the text build on each other and complement rather than contradict each other. A strength of this text is the wealth of case examples. They are important counterparts to the

us

decision to focus on creating a text for practicing clinicians.

The book is separated into eight parts, the first of which focuses on the basic principles of

an

traumatic stress. The first chapter describes the effects of trauma within a public health model, demonstrating the socioecological impact of trauma as well as describing potential routes of

M

intervention. Schnyder and Cloitre provide a comprehensive overview of multiple theoretical

models of posttraumatic stress development, from the cognitive to the psychodynamic. They also included sociocultural, interpersonal, and system level theories, providing readers with the opportunity to understand the biopsychosocial effects of trauma from a multitheoretical conceptual framework—essential for assessment, conceptualization, and treatment planning. The overview of the neurobiological information is comprehensive and describes the effects of traumatic stress on a number of different systems of the body. These chapters also successfully link the effects of trauma to biobehavioral health outcomes such as immune system functioning.

I was also glad that the text described psychological pathways for health-related outcomes as well as clinical practice implications.

The next two sections include a chapter on the diagnosis of trauma-related disorders followed by chapters on treatment. The book presents a thorough discussion of the differences in diagnostic criteria between the International Classification of Diseases and the Diagnostic and Statistical

Manual of Mental Disorders, as well as the diagnostic spectrum evolution over time. The text then provides an excellent discussion regarding the benefits and risks of early intervention, such

of early intervention when applied without appropriate assessment or follow-up. The meat of the book is a thorough and accessible presentation of a wide range of empirically supported

an

treatments for trauma-related disorders. Each chapter has a similar format, which helps the reader compare the treatments. The treatments described in the text include prolonged exposure

(PE), cognitive behavioral therapy (CBT) for trauma, cognitive processing therapy (CPT), eye movement desensitization and reprocessing (EMDR), narrative exposure therapy, brief eclectic

therapy, skills training in affective and interpersonal regulation (STAIR) narrative therapy, and complicated grief treatment. Each chapter describes the treatment's theoretical conceptualization, how to implement the treatment, challenges to treatment due to comorbid conditions, contraindications, empirical evidence supporting the treatment, and a comprehensive case example. All of the chapters include the application of a theoretical conceptualization to inform treatment decisions.

as erroneous beliefs about the vulnerability of trauma survivors and the potential harmful effects

The next three sections describe treatment with comorbid conditions, special populations, and special treatment modalities. Chapters on comorbid substance abuse, borderline personality disorder, and chronic pain each include a summary of important findings, recommendations for current practice, and theoretical reasons for dual presentation. The section of special populations

includes summary chapters on children and adolescents, older adults, refugees and immigrants, and veterans. The three chapters on special treatment modalities provide a discussion of the research support, methodological concerns, considerations for trauma type, and benefits and risks

of group treatment, couples treatment, and telemental health. The seventh section provides a discussion of pharmacotherapy for trauma-related disorders, including the evidence supporting

This section also includes a presentation of treatment guidelines and clinical applications for the use of medication for trauma survivors.

The last section provides a brief overview of treatment selection, including a description of common factors across treatments, treatment matching criteria, therapist competencies (including

multicultural competencies), the importance of client choice, treatment sequencing and

multicomponent therapies, and the need to focus on functional impairment as an outcome measure to determine treatment success. This chapter provides a good closing to the text as it begins to address the enormity of treatment options that are now available for trauma-related disorders.

While this text is better than many others in its accessibility for the practicing clinician, there are still some parts related to neurobiological and health-related outcomes that may be a little

advanced for the average clinician in the field. While I was also impressed with the breadth of treatments included in the text, I do think that a more detailed discussion of the common elements across treatments as well as a chapter dedicated to conceptualization and treatment selection would have improved the book's utility for the practicing clinician. What may have also

helped is the inclusion of a chapter on assessment of trauma-related disorders in order to help practicing clinicians make data-informed choices on treatment selection. I also concur with Schnyder and Cloitre, who state themselves that a separate volume on treatments for children

and adolescents is needed; the same could be said for each of the special populations included in the text.
