The recent report on giant cerebral cysticercosis is noteworthy1 as it draws attention to ‘‘the importance of considering cysticercosis in diagnosis of acute presentations of large cerebral masses in infants, particularly in prevalent regions.’’1 Giant neurocysticercosis can simulate a brain neoplasm pathologically and this can result in misdiagnosis.2 Images may resemble ‘‘multiple hypodense lesions with no contrast enhancement or perilesional edema.’’3 The rapid evolution of giant cysticercosis in the case described is unusual, because cysticercosis is usually a chronic disease in which brain cysts develop over a period of many months or years. In cases of infantile neurocysticercosis, a Taenia carrier is usually identified in the household.4 Del Brutto4 suggested that searching for a Taenia carrier is essential and that ‘‘the search must not be limited to family members, but also extended to domestic employees who are in daily contact with the children.’’ Viroj Wiwanitkit, MD Hainan Medical University, China
