A 45-year-old man presented in shock. Chest radiography showed a large opacity in the left hemithorax (Figure 1). Computed tomography revealed a ruptured lateral wall of the left ventricle with pseudoaneurysm formation (Figure 2). He was resuscitated and taken to the operating room. After sternotomy, the left half of the sternal table was retracted gently. The pericardium over the aorta was separated, and pursestring sutures were placed. Similarly, the right atrium was dissected out and aortic 2-stage cannulation was performed. After instituting bypass, the left half of the sternum was elevated and the dissection was completed.

The pseudoaneurysm was opened after clamping the aorta and venting the pulmonary artery. A Dacron patch was sutured to the large opening in the left ventricular lateral wall. The ventricle was closed with 2 Teflon strips (Figure 3), and the wall of the pseudoaneurysm was sutured over it. The patient was discharged home on postoperative day 8 (Figure 4).
