Global HIV Funding And Local Contexts Victoria Fan and coauthors (Dec 2014) show that the new allocation model for HIV funds of the Global Fund to Fight AIDS, Tuberculosis, and Malaria is expected to significantly lower the allocation shares of some countries while significantly increasing the shares of others. The new methodology, which takes into account a country’s disease burden and ability to pay, also includes discretionary adjustments. Among the countries expected to receive major allocation share cuts, Haiti has not yet recovered from the aftermath of the 2010 earthquake, 1 and Ukraine is currently undergoing major economic turmoil.2 Such situations compromise a country’s ability to pay, submit timely and high-quality requests for funds, and find and make use of other funding. Hence, donors should examine the potential negative impacts, considering countries’ changing economic situations, before deciding to reduce their funding shares. It has long been argued that when the principle of utility (maximizing benefits) and the principle of equity (the fair distribution of resources) are in conflict, policy makers should allo-

cate resources in an ethical and transparent manner, taking local circumstances into account.3 The new model is a welcome step. However, reducing the funding share of countries that are undergoing exceptional economic hardship requires a close assessment of both changing local conditions and disease burden. Failing to do so could be counterproductive to the global efforts to fight HIV. Mahesh Devnani and Anil K.
