Modus operandi.?Chloral hydrate hypodermically acts by relieving the arteoles of their pressure of blood, and thus by dilating the vessels the blood is permitted free motion. Consequently the vessels become full and the skin rosy and warm. Chloral hydrate also acts as a sedative to the vaso-motor nerves, and thus dilates the vessels. The internal viscera are thus relieved from congestion?due to impaired circulation of blood. Calcutia, 27th September 1875.

called on the night of the 20th instant to treat two of cholera in a family in the vicinity of the town. I saw a boy and a girl attacked with the disease. They were both in the collapsed stage; extremities icy cold; eyes half shut; passing stools in bed clothes unconsciously ; no pulse at the wrist; and temperature below par. The boy and the girl were found restless and craving for iced water constantly (unquenchable thirst). On enquiry I learnt that they were under regular treatment from the beginning, getting astringents, and stimulants regularly ; but the stomach became so irritable, the moment the medicine was administered it was brought up the very next moment. I at once proposed to give chloral hydrate hypodermically. I injected 2 grains of chloral hydrate in solution to each of the patients in the arms at point of insertion of the deltoid muscle, and half an hour after I gave a second injection, as the first produced no effect on the system. Again after the second half hour, finding no good, I injected the third time, and, to my utter satisfaction, I found the cases had tuken a good turn. I ordered iced water and brandy, a tea spoonful every half hour till the whole body became warmer, and arrowroot at intervals. 21s??Morning.?I found the patients much better; pulse good ; body of normal temperature ; made no water ; no inflammation at the place of injection. I stopped brandy water and ordered milk and arrowroot ad-libitum. Evening.?No motion ; made a few drops of urine of high colour. Doing well in all other respects. 22nd.?Morning.?A dose of castor-oil with 3 drops of tinct. opii to each of the patients. Evening.?Had 3 stools from the oil; both the patients doing well ; appetite good.
