Inaccuracy in factor VIII measurement due to administration of recombinant activated factor VII in vivo

Introduction Recombinant activated factor VII (rFVIIa) is a bypassing agent used in haemophilia A patients with factor VIII (FVIII) inhibitors. The mechanism of rFVIIa is FVIII independent, as it is thought to improve factor X activation either through a tissue factor-dependent or a platelet surface pathway [1]. We present a case in which in vivo rFVIIa administration led to inaccuracy in FVIII measurement based on the activated partial thromboplastin time (APTT).

Case Report The patient is a 9-year-old Congolese male immigrant. In April 2012, the first sample from this patient was sent to the Finnish Red Cross Blood Service haemostasis laboratory. With the APTT-based one-stage clotting assay,
