Abstract MRI is an excellent imaging modality for the evaluation of acute hip pain in the ED and provides accurate diagnosis; facilitating appropriate and timely management thereby can result in decreased cost and improved prognosis in these patients. In this article, we review the use of MRI for patients presenting with acute hip pain in the Emergency Department (ED), including MRI protocols, MRI features of common acute hip pathologies, pivotal differential diagnoses, and management options. Introduction Acute hip pain in the Emergency Department can be generally classified into two categories: traumatic and nontraumatic

pathology which includes infectious, noninfectious inflammatory, and miscellaneous subtypes. Clinical history and radiography or computed tomography (CT) are sufficient in diagnosing cortical-based conditions of the hip; magnetic resonance imaging (MRI) is the imaging modality of choice for comprehensive evaluation of the soft tissues, muscles ligaments and bone marrow, thereby substantially improving diagnostic accuracy. Although radiography is typically the initial imaging technique for evaluation of acute hip pain patients, “occult” fractures and soft tissue injuries, may not be visible on radiographs, thereby delaying diagnosis and treatment with resultant increases in morbidity and mortality [1]. Ultrasound (US) may be helpful in assessing soft tissue structures but is operator dependent and has limited diagnostic value for osseous pathology. CT imparts small risks of radiation exposure and provides limited evaluation of muscles, ligaments,
