Melorheostosis of the spine and ribs A 70-year-old male patient presented to our hospital with complaints of cough and sweating. He had been diagnosed with type II diabetes for the past 5 years. On lung examination, crepitant rales were found; the other systems were normal. The laboratory findings showed hyperglycemia and mild leukocytosis. Posteroanterior chest

radiography and thorax computed tomography were performed to identify the lung infection. On computed tomography, bilateral lung consolidation suggestive of pneumonia was detected. On chest radiography and computed tomography, a large hyperostosis area was detected in the left ribs and cervical and thoracic vertebrae (Figure, Top and Bottom). The imaging findings were consistent with melorheostosis.

Melorheostosis is an unusual mesenchymal dysplasia that commonly presents on radiographs as longitudinal bars of hyperostosis in osseous structures. Serum calcium, phosphorus, and alkaline phosphatase levels have been reported to be within normal limits in melorheostosis.
