PROCEEDINGS OP THE MADRAS GOVERNMENT, PUBLIC DEPARTMENT, 25th FEBRUARY, 1867. No. 127. Read the following despatch from the Right Honorable the Secretary ot State for India, to His Excellency the Right Honorable the Governor in Council, Fort St. George; dated India Office, London, 24th December, 1866, No. 46. I have considered in Council the despatch from your Government, dated 9th October, (No. 34) 1866, forwarding, with its order on the subject, a copy of a minute recorded by your Excellency on the condition of the Civil Dispensary at Coimbatore, which you visited when recently passing through that station. The state of the dispensary appears to reflect the greatest 2. discredit on the officers connected with it, as well as on the Civil Officers of the District, who are very justly expected to exercise a general supervision over those institutions ; and I regret to observe that your Excellency, from what you witnessed on your recent tour to the north, had been led to form an unfavorable opinion as to the management of the dispensaries in that quarter of the presidency. 3. Her Majesty's Government have noticed with much gratification the gradual extension of opportunities of improved medical treatment to the poor inhabitants of the Mofussil, by means of dispensaries established by the joint action of Government, and of the local community, and they look forward to a still greater extension of the system, as the natives become more sensible of the advantages of the European system of medicine. In order, however, that the dispensaries shall be maintained in a state of efficiency, and be made the means of the largest attainable good to the people, it is necessary, on the one hand, that the native inhabitants of the towns where the dispensaries may be established should take an active interest in their management, as well as provide for those expenses which are not undertaken by the Government, and, on the other hand, that the Civil Officers should exercise such a supervision over the dispensaries as will ensure a careful and efficient discharge of their duties by the officers immediately connected with their management. 4. Your orders of the 5th October will probably be found sufficient to lead to a careful discharge by public officers in the provinces of the obligations incumbent on themselves, individually, in regard to the dispensaries ; but it seems to me desirable to direct the special attention of the Collectors and other Civil Officers to the importance of enlisting the sympathy and active co-operation of the native community in the actual working of the dispensaries. I am of opinion that means should be taken for ensuring truthfulness of the reports, and the accuracy of the returns furnished annually by the Medical Officers of different grades in charge of the dispensaries, and I would suggest that the Civil Officers should be required to certify to your Government their opinion as to the correctness of the annual reports, and as to the general state of the dispensaries. 5. the

No. 370 of 1867.?In continuation of Government General Order No. 1060 of 23rd December 1864, and in accordance with instructions received from the Right Hon'ble the Secretary of India, His Excellency the Governor General in Couneil is pleased to lay down the following revised scale of consolidated salaries for Officers of Her Majesty's Indian Medical Service. 2. The number of Medical Officers allowed for each Presidency Town with its immediate suburbs, and the allotment of duties amongst them, are based on the recommendations of the Commission appointed to take this subject into consideration ; hut it will be open to the local Governments to modify the to proposed allotment of duties, in such manner as from time time may seem most desirable, with reference to the convenience of the service, provided the prescribed number of Officers aud the aggregate of salaries is not exceeded. 3. The appointments of Presidency Surgeons in Calcutta ?will eventually cease to exist, the duties being performed by the several Medical Officers holding appointments at the Presifrom private practice. These dency who are not debarred Officers will be required to attend those servants of Government who may seek their services; and also entitled to gratuitous aid, to serve in rotation as Members of Standing and other Medical
Committees.

6. Medical Officers holding appointments at the Presidency, and not provided with public quarters, will be granted Presidency house-rent according to relative rank, and on the scale granted to Officers of the Staff Corps holding appointments at the Presidency Towns. The grant of Presidency house-rent is limited, however, to salaries not exceeding Us. 1,400 per mensem. 7. The allotment of Civil Stations and Residency charges to their respective classes will be notified hereafter. 8. In order to render this scale of Indian Medical salaries more complete, the salaries of the administrative appointments and regimental charges already provided for in Government General Orders No. 1060 of 1864 and No. 901 of 1866, have been ineluded. Officers of the Indian Medical Service will be on the as Military Staff Officers as regards pay and allowances to be drawn while absent on leave,?the pay of their rank, 29 of Government General Order No. as laid down in para. 1060 of 23rd December 1864, being in all cases taken as the basis of the calculation of the moiety due to the absentee and to the Officer officiating,?present incumbents being allowed the privilege of receiving, during such absence, either the allowance thus due to them under the Military Regulations under the revised scale of regimental pay and staff salary, or that to which they would have been entitled under the old scale both of pay and staff allowance. 10. The scale of salaries now sanctioned will be brought into operation from the 13th of November, 1866, the date of the receipt of the instructions of the Right Hon'ble the Secretary of State, and be made to apply in all cases where there is no reduction in the present aggregate salary. 9.

Present incumbents, whether drawing the old regimental rate of pay with, or the new regimental rate of pay without, staff salary, will have the option of retaining such rates, if more beneficial than those now ordered, except in cases where the salaries have been specially fixed, pending the revision now brought into effect.
