• A 26-year-old serviceman was admitted to Wilford Hall US Air Force Medical Center, Lackland Air Force Base, Texas, because of chills, fever, diarrhea, and arthralgias of ten days' duration. He had no history of gallbladder disease, alcoholism, or pancreatitis and no family history of the latter. He had become ill while returning from a training flight to central Africa and had been treated for five days without improvement at another hospital. During the five-day hospitalization, the patient had daily spiking temperatures ranging from 105 to 106 F. A pleural effusion on the right side of the chest yielded 1,000 Somogyi units of amylase and 3.2 gm of protein and 112 mg of glucose per 100 ml. Other findings were mild dehydration, hepatosplenomegaly, and epigastric tenderness. Laboratory test values were: hematocrit 38%, WBC 4,860/cu mm, SGOT 113 mU/ml (normal up to 38 mU), lactate dehydrogenase (LDH) 190 units/ml, serum amylase 350 Somogyi units/lOO ml, total serum bilirubin 2 mg/100 ml (1 mg direct), and serum alkaline phosphatase 24 mU/ml (normal
