It can also be used as a core list by small hospital library consortia. Books and journals are categorized by subject, with the books being followed by an author index and the journals by an alphabetical title listing. Items suggested for initial purchase by smaller libraries are indicated by an asterisk. To purchase the entire collection of books and to pay for annual subscriptions to all the journals would require an expenditure of about $22,500. The cost of only the asterisked items, recommended for first purchase, totals approximately $6,100.

SINCE the inception of this "Selected list of books and journals for the small medical library" in 1965, enormous demands have been imposed upon the small medical library, and it has evolved dramatically into a fundamental component of the vast biomedical communications network. Fourteen years ago, collection building was the vogue, and often it was done with little direction or purpose, especially when ample money was available. Because at that time many small hospital library collections were being built and updated, there was notable need for selection tools to guide hospital librarians in assembling useful collections of quality and not necessarily quantity. To help meet that need, the first version of this list was compiled to serve as a selection aid in a small medical library of a hospital, society, clinic, or similar organization. *An pighth revised version of a paper originally published in the Bull Med. Presently, collection analysis and development have replaced collection building in libraries, and the small medical library is no exception. The concept of cooperative resource sharing has become a reality. In the 1970s austere budgets, galloping inflation, and accentuated demands for access to biomedical information have all contributed to the priority of high selectivity in acquiring library materials. The present list, like those preceding it, is primarily meant to be a guide for the librarian of a small health sciences library in collection development and analysis. It can also be used as a core list for a consortium of small hospital libraries. Audiovisuals have not been included in this list because, as yet, standards, preferences, and needs for nonprint materials vary widely from institution to institution and from staff to staff. Therefore, the best advice on acquiring audiovisuals can be obtained from the multimedia center of a large biomedical institution. The emergence of the hospital library as an educational and informational entity has been due largely to the National Library of Medicine Regional Medical Library (RML) Program and, to a lesser extent, to the Joint Commission on Accreditation of Hospitals (JCAH). In order to carry out its function in the RML Program, a hospital library must have an up-to-date basic book and journal collection to fill the immediate needs of its staff. Cooperative resource sharing and multi-institutional library consortia have been advocated by the RML Program. For a number of years JCAH has recognized the responsibility of an accredited hospital to provide adequate professional library services to its staff, but the JCAH criteria for achieving this ultimatum have been general and somewhat vague. Because the standard has been relative to the needs of the individual institution and because no specific guidelines have been set forth, the interpretation of "adequate professional library services" varies

widely, depending upon the institution, from excellence to minimal mediocrity. However, in the 1979 edition of the Accreditation Manual for Hospitals [ 1], JCAH has presented a significantly stronger principle than ever before relating to professional library services in a hospital: "The hospital shall provide library services to meet the informational, educational, and when appropriate, the research-related needs of the medical and hospital staffs." Much more than a few books and journals has to be provided. The first standard listed under this principle deals with appropriate supervision, staffing, and resources of the library. In order to assure that the first is utilized properly, a second standard has been mandated, which requires that the provision of professional library services be guided by written policies and procedures; no such requirement existed previously. Up to this point, JCAH had given a rather generalized interpretation of its principle and standard for the hospital library. Now, that interpretation has become considerably more specific while still being based upon the needs of the institution, but additionally, upon the needs of its cooperative resource-sharing arrangements with other libraries and informational systems. Criteria for qualified, knowledgeable personnel are clearly stated, and the service of a medical librarian on a full-time, part-time, or consultative basis is compulsory. A current collection of authoritative print and nonprint media to support clinical, educational, and research activities is essential. Goals for the hospital library and sources of funding to achieve those goals will have to be stated in writing. In fact, the interrelationship between the library and the other hospital units has to be shown in the overall hospital organizational plan, thereby giving status to the library as a full-fledged unit itself. By having required written policies for acquisitions and collection maintenance under the new JCAH standard, the quality and relevance of library resources should notably improve in many hospitals. In a similar way, written policies concerning levels of reference, bibliographic, and access services provided by the library will be of value to both the librarian and clientele in maintaining such services at the prescribed levels. By these standards, JCAH is, in reality, stating that the hospital library is part of a larger informational network and no longer merely an entity unto itself. Hopefully, JCAH will continue to upgrade its standards for the status of the hospital library and will give even more specific interpretations to 186

them. Presently, there is little in the literature pertaining to the hospital library that is written specifically for the hospital administrator. Likewise, there are still few up-to-date selection aids to guide the hospital librarian in developing a substantive collection to meet the needs and objectives of the library. Such help is especially needed now, when the quality and relevance of the book and journal collection are paramount and fiscal stringency is an ever present reality. The trend of escalating informational demands upon the small medical library shows no evidence of subsiding. Instead, the shrinking library book and journal dollar cowers before the expanding requirements of continuing education, recertification, and relicensure. Equally as important, ongoing clinical and research needs of the staff must be met by the hospital library. Effective book and journal selection in the small medical library is very difficult, for high selectivity is imperative and funds are usually limited; therefore, educated judgment is essential. Nearly ten years ago Stearns and Ratcliff presented the "core library" concept [2]. This list was purported to be the absolute minimal collection necessary for a hospital library. Even in the early 1970s, the Stearns-Ratcliff core list indeed proved to be minimal and restrictive in scope so that only for the smallest hospital libraries did it even approach adequacy. Their core list still has not been revised and is obviously outdated. However, some of the Regional Medical Programs and RMLs have compiled more recent core lists to be used as selection guides by hospital librarians in their respective areas. In view of the upgraded role of the hospital library during the past decade as a biomedical information resource, a core list hardly meets needs of the typical hospital library and must be supplemented. While the core lists are inadequate for most hospital libraries, book and journal lists compiled primarily for practicing physicians tend to be too extensive in the clinical areas and too sparse in basic sciences and nursing to fulfill the needs of the typical hospital library. Nevertheless, when indepth coverage is required in selected clinical specialties, such a listing as "A library for internists III" [3] is an excellent, up-to-date selection tool, and it will be revised on a triennial basis. "Books in clinical practice 1971-1975" [4] is another eminently good list of clinical books for the period specified, but unfortunately it is somewhat outdated. The Library Association has recently issued a new edition of Books and Periodicals for Medical Libraries in Hospitals [5], a subject listing of publications which a subcommittee of its Medical Section considers most likely to provide useful coverage in various basic science and clinical fields. There is no preference evaluation given to the publications listed in this selection guide. It is British in origin and viewpoint; that factor should be considered when one uses this guide, because a preferred medical text in the United Kingdom is not necessarily accepted as such in the United States. But, it is also true that a number of publications having British origin and marketed under imprints of American publishers are recognized medical standards in the United States. As previously stated, our selected list of books and journals has been put together specifically for the librarian of a small medical library in a hospital, clinic, society, or other health care facility. We have attempted to provide a well-balanced, current collection of English-language books and journals both in clinical and basic sciences, while keeping in mind high quality and maximum use of the library dollar. The list has been revised every two years since 1965, with this one being the eighth update. Over the years, as the inflationary spiral has accelerated, more expensive publications and fewer inexpensive ones have been listed simply because in some disciplines inexpensive quality publications no longer exist. Since this list can be used as a consortium core list, high-priced publications could be obtained on a cooperative resource-sharing basis. When expensive publications are noted, the hospital librarian using this list still has a choice, whenever possible, of less expensive alternatives in the same subject field, providing they meet our qualitative specifications. Emphasis has been placed upon selecting the most recent authoritative works in varying price ranges. The recommenda-

tions and suggestions of both subject specialists and biomedical librarians have been taken into consideration as we have assembled this list. In many instances, information regarding current prices and editions has been supplied by publishers and distributors. Price information quoted in this list is as accurate as we could obtain it at the time of publication, but undoubtedly many of these prices will increase in a short period of time if present trends continue. The approximate total cost for purchasing all of the books and for entering one-year (1979) subscriptions to all of the journals in this list amounts to $22,500-$17,000 for books and $5,500 for journal subscriptions. Because many libraries cannot or may not want to buy all or even most of the listed publications, items suggested for priority purchase have been indicated by an asterisk. Purchasing only the asterisked books and journals would entail an expenditure of about $6, 100$4,200 for books and $1,900 for one-year journal subscriptions. Book and journal prices continue to increase at a substantial rate (Table 1). The figures given in Table 1 are based exclusively on prices for publications that have been in previous versions of this list and on prices for those that are in the present one. In 1965, the average price of a journal subscription was $13.90, and in 1979, $40.27, thereby reflecting a cost increase of 189.7%. The average price of a book has risen by 131.7% from 1965 ($15.00) to 1979 ($34.76). The averag book prices, especially in the 1975, 1977, and 1979 lists, are not totally representative for the clinical and basic science books, because the average has been lowered by the disproportionately large number of nursing books, which are considerably less expensive. For example, in this revision, the average price of all the books is $34.76, but when the nursing books are excluded, it jumps to $38.04, while the average

price of the nursing books as a group is $14.68. Again, we are reemphasizing the fact that the figures stated here are applicable only to the small medical library collection and are not indicative of the much higher book and journal costs borne by the large biomedical library. Even though we believe that the list of books and journals we present on the following pages represents high selectivity and quality, we in no way consider it to be the ideal collection for every small medical library or even for most of them. It should be used as a selection aid and be related and adapted to the activities, objectives, and dominant subject specialties of the particular institution of which the library is a part. Collection development is an individual process, for each library has its own unique needs. Our list does not represent a complete coverage of all high-quality literature in the various health sciences disciplines, and in most libraries the list should be supplemented by additional books and journals in subject areas of greatest interest to the library clientele. A very small hospital library collection can be greatly enhanced by including publications of national professional associations in the health care field. These publications are generally authoritative in scope, usually inexpensive, and tend to be very heavily used by hospital library patrons. The publication catalogs of such organizations as the American Dental Association, American Hospital Association, American Medical Association, Catholic Hospital Association, and National League for Nursing are worthwhile acquisitions tools for the hospital librarian. The American Hospital Association annually issues a selected guide to current literature, books and periodicals, in hospital and health care administration titled Administrator's Collection [6]. While the typical hospital library would not need the in-depth collection presented in this guide, it does provide an invaluable source of information to the hospital administrative staff and to the hospital librarian regarding quality materials available on the subject, and it should be considered as an essential acquisitions source in the hospital library. The hospital librarian, especially the inexperienced person, faces not only a paucity of selection guides but also a dearth of practical published information on ordering informational materials, organizing the collection, providing reference service, and interlibrary loan procedures. Library Practice in Hospitals: A Basic Guide [7], which was written especially for the inexperienced hospital librarian, is a useful reference for all hospital 188

librarians. Its chapters on selection and acquisitions provide basic concepts with which the hospital librarian should be familiar when using any selection aid. More advanced information is presented in comparable chapters in the Handbook of Medical Library Practice [8]. Both of these standard works are now outdated but fortunately are in various stages of revision. The Pacific Southwest Regional Medical Library Service has recently issued the fourth edition of its Manualfor Librarians in Small Hospitals [9]. This manual is also directed to the inexperienced person in charge of a hospital library and contains a wealth of practical, current information in a concise, accessible format. Its scope is not limited to the southwest region but is relevant to hospital libraries throughout the country. Because of the enhanced position of the hospital library in the biomedical communications network, the hospital librarian should consider the Bulletin of the Medical Library Association as an indispensable source for current awareness and professional development. For health sciences librarians who are participating in programs of patient education, Medical Books for the Layperson: An Annotated Bibliography [10], compiled by the reference librarian from Science Reference at the Boston Public Library, is a helpful selection guide on a variety of medical and health-related subjects. It includes books through mid-1975; later publications are listed in a supplement issued in 1978. Since patient education is becoming an increasingly important subject both to health care personnel and to patients as well, this book is especially pertinent at this time. Due to the upgraded stature of the hospital library within its institution, reference service and the reference collection have taken on new importance and sophistication. "Selected reference aids for small medical libraries" compiled by Duncan [ 11 ] has been considered the standard list of reference works for the hospital library, but it was published early in 1970 and is presently outdated. A current source for similar information is Reference Materials for a Health-Science Core Library [ 12], put out by the Pacific Southwest Regional Medical Library Service. This manual attempts to provide the hospital librarian with basic sources of information and includes an annotated bibliography of selected, heavily used dictionaries, directories, handbooks, drug references, and audiovisual catalogs. The Medical Library Association has scheduled the 1979 publication of a book titled Introduction to Reference Sources in the Health Sciences [ 13], which should Bull. Med. Libr. also offer much needed help to the hospital librarian in the development of a reference collection. In our selected list of books and journals, we have not attempted to make a systematic coverage of reference sources for the small medical library and have included only a few of the most obvious ones. In the book section under Pharmacology and Therapeutics, we have included three technical subject reference works, namely, The Pharmacopeia of the United States of America, The National Formulary, and The United States Dispensatory, because in the United States they are the most widely recognized reference sources for pharmaceutical standards and for therapeutic drug dosage, with the first two having been given legal status by the Federal Food and Drugs Act of 1906. In the 1980 revision, The Pharmacopeia of the United States and The National Formulary will be consolidated into one publication and issued by the United States Pharmacopeial Convention. In hospitals where these drug references are purchased for the pharmacy or pharmacology department, it may not be necessary to duplicate them in the library. In any case, the hospital librarian should know where they are housed in the institution and have recourse to them for reference purposes. For hospital libraries serving institutions having extensive dental programs, the book section on Dentistry should be supplemented by a "Selected list of books and journals for a small dental library" [ 14]; a 1979 revision of the list is planned. The Bureau of Library Services of the American Dental Association has compiled an annotated bibliography titled Basic Dental Reference Works [15], which is a very good selection tool for dental reference materials. In this list, as in the previous one, we have included a disproportionate number of nursing books because presently there exists no recent comprehensive selection guide to assist nursing school or hospital librarians in their choice of informational print materials in the field of nursing. For many small medical libraries, the number of books in the Nursing section is excessive, and the asterisked items will fill their needs adequately. The books included in this section have been chosen because of their high caliber in presenting the most up-to-date aspects and concepts of the nursing profession, as well as in setting forth sound clinical procedures. The Nursing book section, as it appears, was compiled for the hospital librarian whose library supports a nursing curriculum or for the inexperienced nursing school librarian. While Bull. Med. Libr. it is not adequate for a nursing school library, it does provide a broad base on which to build an inclusive collection of nursing books. However, we are now in the process of preparing a comprehensive list of nursing books and journals and hope to make it available sometime during the current year. The Interagency Council on Library Resources for Nursing biennially revises its "Reference sources for nursing" [16], and we have made no effort to include nursing reference items in this list. This is another source that a hospital librarian can use in developing the reference collection since it is not limited only to nursing reference items but also includes standard biomedical reference works as well. Basic science journal literature is becoming more important to the hospital library as its role in continuing education expands. Because of copyright restrictions, access is becoming more difficult unless such journals are held at the local level. Institutional needs vary greatly depending upon research and educational programs. Because some of the basic science journals are among the most expensive items in a hospital library, their purchase should be weighed against their use. Access is essential, but individual acquisition is not always cost effective. Purchasing such items on a cooperative resource-sharing basis with other local hospitals in either a formal or informal consortium arrangement provides both accessibility and economy. Methodology for journal resource sharing is discussed in "A cooperative serial acquisition program: thoughts on a response to mounting fiscal pressures" [17]. In most hospital library situations, ordering books and journals directly from the publishers is an inefficient and expensive use of manpower, for it involves a massive amount of paperwork in generating and handling multiple purchase orders and invoices. A dependable medical book dealer, preferably in one's own geographic locale, and a reputable subscription agent, serving as primary sources for books and journals, can alleviate a great deal of paper work, as well as many of the problems resulting from dealing with publishers directly. Both the medical book jobber and the subscription agent are above all else selling service, and the standard of service should be the determining factor when the hospital librarian chooses these vendors. The most valid information for evaluating either medical book dealers or subscription agents can be obtained from medical and academic librarians in the surrounding area who deal with them as consumers. in order to obtain optimum service from any vendor, it is necessary that accurate, complete order information be submitted by the library. Usually, reliable medical book dealers offer a 10% discount on books from major medical publishers in the United States. There are medical and scientific publishers who no longer discount on retail orders sent directly to them by health sciences and academic libraries. While it is customary for medical book dealers to discount on the books they sell, most subscription agents now charge service fees ranging from 4% to 10%. Even so, the charge is still generally less than the expense of labor costs that result from entering subscriptions on a direct basis. A low service fee does not mean a monetary saving for a library if the agent's service is substandard; in reality it can be expensive. The ability of a subscription agent to follow through during the year and effectively claim missing journal issues should be considered as a primary criterion when the decision is made to use the services of such an agency. Even though, as we have mentioned before, this list is basically intended to be a selection aid for hospital librarians, previously published versions have been used extensively by medical and

academic librarians, by physicians for personal book selection, and in teaching programs. It can also serve a useful purpose in the public library acquisitions program, because medical books and journals are no longer the exclusive bastion of the physician. Due to consumer movements, media exposure, malpractice suits, and a generally more enlightened public, demands for professional medical literature in the public library are increasing tremendously. In many instances, medical books written specifically for the layman no longer fill the need of many individuals seeking information on various medical and related matters, and access to medical libraries is not usually available to the public [18-19]. The multiplicity of medical malpractice cases has made medical literature an integral part of the professional life of the attorney dealing with such cases. An "Attorney's guide to medical literature" was published in the American Journal of Law and Medicine in the fall of 1977 [20], and three years before, Medicolegal News issued a list of "'Authoritative' medical texts" [211]. Our list can be used to supplement and complement such guides when it is necessary to acquire medical publications in a law library or when an attorney must sort out the massive amounts of medical literature in order to find standard, authoritative, recommended works. In the latter instance, previous versions of this list (see footnote, p. 185) can be useful in determining such publications for earlier years because many malpractice cases involve medical treatment administered a number of years prior to the onset of legal action. For the purpose of keeping this list current for as long a period of time as possible, in instances where a new edition of a standard work is scheduled to be published in 1979, it is that edition which has been cited here. On occasion, the previous edition may still be in print, and if the book is needed immediately, bibliographic data for the in-print edition may be found in the 1977 list (see footnote, p. 185); price information should always be verified. If a new edition has been planned for 1980, the current in-print edition is noted with a reference to the upcoming new one. Textbooks now out of print or outdated that have appeared in previous lists have been replaced by recent works of corresponding status in their respective fields. When a book has more than two authors or editors, the notation "[and others]" is made following the name of the senior author or editor.
