Since 1905 when Cotton first reported a case of traumatic rupture of the testis, there have been approximately 50 cases reported in the medical literature.' The true incidence of this condition is unknown, although it is likely that traumatic testicular rupture is underdiagnosed and often treated con~ervatively.~>~ Delayed repair of testicular injury may lead to impaired fertility related to testicular loss. CASE REPORT

The patient, a 21-year-old college student, presented to the emergency room after sustaining a kick to the groin during an assault a few hours earlier. He was in mild distress, and complained of right scrotal pain and swelling. An ultrasound examination demonstrated disruption of the tunica albuginea with extrusion of portions of the disrupted testis and areas of hemorrhage (Figures 1-3). The patient was taken to the operating room for emergency right scrotal exploration.
