136. Indikationswandel in der chirurgischen Behandlung der Spondylitis tuberculosa E. Carstensen Krankenhaus Mtihlenberg, Malente Spondylitis Tuberculosa - Local Operation and Physiomechanic Therapy
Summary. The changes in the treatment of spondylitis tuberculosa over the last 10 years are traced with reference to 856 cases. The conservative treatment applied from 1952-1964 (gypsum bed and corset) led to Pott's disease. Since 1964 the emphasis on surgical treatment of abscesses and focuses of infection as indications of local spine destruction with subsequent physiotherapy has prevented Pott's disease and shortened the duration of treatment from the former average of 950 days to approximately 100 days. Die konservative Behandlung von 1952-1964 mit Gipsbett und Stiitzkorsett fiihrte in der Regel zu einer ausgepr~igten Gibbusbildung. 137. Fusionsoperationen an der instabilen Halswirbels iule G. Muhr, H. Tscherne und R. Reschauer Urffallchirurgische Klinik, Medizin. Hochschule Hannover Fusion for T r e a t m e n t of Instability of the Cervical Spine
Summary. Painful functional disorders and the risk of neurologic injuries make the surgical management of unstable cervical spine by fusion essential. Fusion is also recommended in cases of tumors and rare cases of paraplegia. A description of the technique with a posterior approach between C1 and C3 and fusion by means of a bone graft and wire loop is followed by a demonstration of anterior stabilization with bone grafts, graft and plate, or plate and bone cement. Finally, the results and complications observed in 24 patients with traumatic or posttraumatic disorders or tumors are discussed.
