The current financial status of the health care industry is viewed both from its effect on the hospital library collection and the response of the hospital library to the financial crisis. Predecessors of this list have been intended as selection guides for a small or medium-size library in a hospital or comparable medical facility. As the prices of books and journals continue to soar, the secondary purpose as a core collection for a consortium of small hospital libraries or a network sharing library resources may eventually become its primary use. Books (607) and journals (140) are categorized by subject; the book list is followed by an author/editor index, and the subject list of journals by an alphabetical title listing. To purchase the entire collection of books and to pay for 1991 subscriptions would require about $77,700. The cost of only the asterisked items totals $29,300.

BACKGROUND Are we perceiving the hospital library as a bibliographic feast in a time of financial famine? Are we attempting to apply Industrial Age standards to an Information Age situation? Does the hospital library have a future in the health care environment of the 1990s and beyond? These are questions that have pervaded our thinking as we compiled the list of books and journals that follows. In view of the soaring cost of medical literature and the shrinking health care dollar, we frequently ask ourselves if all our work on this project is worthwhile. Is it still relevant to the hospital librarian at a time when we cannot even define what the hospital library should physically be? The ever-rising cost of health care is the top concern not only of the health care system but of American business leaders as well [1]. As a result of the dramatic changes in federal funding policies in the 1980s, the health care industry is still reeling under growing financial and social pressures as it attempts to adjust to new roles and new realities. tolls that in many cases were shared, sometimes inordinately, with their libraries. A 1988 national survey of 127 hospitals conducted by the Medical Library Association (MLA) showed that 65% of the libraries reported major negative changes including budget cuts, layoffs, general reorganization, downsizing, and hiring freezes [2]. A survey of Michigan hospital libraries produced similar data for the hospital libraries of that state [3]. Against this bleak backdrop of the 1980s health care environment, hospital librarians learned from the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the Health Care Financing Administration (HCFA), and the New York State Department of Health that their libraries were really not considered very important in the provision of health care. In 1985, the Accreditation Manual for Hospitals (1986 edition) came out in a new format which used asterisks to designate key requirements that were fundamental to the accreditation process; neither of the two standards for the hospital library was so designated [4]. A year later, 1986, a bureaucratic decision from HCFA no longer required the presence of a 195

library in a hospital that received Medicare/Medicaid reimbursement. The New York State Department of Health got on the bandwagon in 1989 and eliminated the requirement for a hospital to maintain a library because, in its opinion, no cost-and-effect relationship could be demonstrated between the library and acceptable patient care [5].

Change constitutes survival in the modern day health care field, and the vast majority of hospital librarians have already recognized that fact. But there are some who find it difficult to imagine a different way of working. WHITHER THE HOSPITAL LIBRARY?

In the current literature of health care administration, four themes recur with notable frequency-survival, change, cost containment, and competition. While upcoming years appear to be as equally grim and uncertain as the past decade, one thing is very certain-there is no more money, and hospital administrators no longer hold out their hands toward Washington for a bailout. The message is that hospitals must learn to provide quality service while living within their budgets. Hospital librarians have already had years of experience when it comes to innovative economizing. Some hospitals will fail outright. Mergers will lead to the consolidation of the hospital field, with strong hospitals acquiring the weaker ones. Such conglomerates will in all probability reduce duplication of services, and this is a reality of which hospital librarians must be aware and with which they must contend. Most experts, optimists and pessimists alike, do believe that our health care system will survive, with the 1990s being a period of transition to new forms of delivery and new forms of financing. Hospitals that survive and become stronger during this transition decade are those that can quickly and willingly respond to change. To sum it up in the words of one medical center executive, "Hospitals that remain successful into the year 2000 and beyond will be those that change before they have to" [6]. This admonition is as true for the hospital library as it is for the hospital. So what does all this mean for the hospital librarian and the hospital library? It means, has meant, and will mean many things that collectively can be identified as a single entity-change-that insidious something that will happen with you if you are prepared to manage it effectively or will happen to you if you are not. It seems to us that the road of hospital librarianship is diverging into two paths. One is paved with challenge, enthusiasm, innovation, creativity, and high hopes. Hospital librarians on this path are experiencing a new vitality in their profession and are anxious to be a part of the Information Age. They realize that the health care industry is looking toward information technology as a means of survival and that information is the business of the hospital librarian. The other path is paved with uncertainty, pessimism, kainophobia, and status quo. For those traveling this path, being a hospital librarian is difficult, tedious, and unappreciated. Their primary concern continues to be founded on books and journals, while the health care industry is focusing on information. All too often over the past three decades, health sciences librarians have tended to equate the terms "computer" and "information" as being synonymous. A distinct polarization resulted, which fortunately is showing signs of disintegration. At one extreme there have been the staunch supporters of automation who have viewed the library as a collection of computers, keyboards, and monitors and have been waiting for years for the printed page to disappear into oblivion. At the other end of this spectrum have been those individuals who have felt compelled to protect the book and journal from the ominous onslaught of automation. Intellectually, none of this makes much sense, but it has continued for a long time as an emotional encounter of futurists challenging the old guard. Technological glitz and the love of the book have been at odds with each other far too long. effects when everyone finally accepts the fact that pulling together is the only route for survival.

Technological glitz and the love of the book have been at odds with each other far too long. Auspiciously for all health sciences librarians, and especially for hospital librarians, priorities seem to be getting back on track, replacing anxiety with a clarity of purpose. If there is any group of librarians who know about survival, it is hospital librarians! But instead of merely surviving the blows of the 1980s, by the end of the decade the hospital library was again being recognized for its unique place in and contribution to the biomedical information network by both its own professional association and other groups as well. In December 1988, MLA's Board of Directors established an Ad Hoc Committee on the Position of Hospital Libraries for the purpose of developing methods for meeting changes in the health care field that affect hospital libraries. MLA has an ongoing dialog with JCAHO to focus attention on hospital information services and is pursuing the issue of compliance in relation to library requirements for accreditation of hospitals. The advocacy of the hospital library was designated as a 1990 MLA priority [7]. Perhaps the most exciting and significant development for hospital libraries occurred in 1989 upon publication of the 1990 Accreditation Manual for Hospitals, which listed the standard for professional library services as a key (asterisked) accreditation item for the first time [8]. In view of the adverse decisions of HCFA and the New York State Department of Health regarding hospital libraries, this JCAHO decision has served as a much-needed morale boost for hospital librarians. Demanding that this standard be enforced is not only the right but also the responsibility of MLA and hospital librarians alike. JCAHO has a major program, "Agenda for Change," to review and revise the hospital accreditation process. It was during the preparations for revising hospital standards to ensure quality patient care that JCAHO decided that improvement in patient care would be encouraged by and may be dependent upon better information management by health care institutions [9]. In 1990, the American Hospital Association (AHA) conducted a comprehensive survey of health sciences libraries in all hospitals in the United States registered by AHA. The purpose of this survey was to provide baseline data on the current state of libraries in hospitals, since hospital librarians have little data Bull Med Libr Assoc 79(2) April 1991

about their own group of libraries. The AHA undertook this survey because hospital librarians have been vocal in expressing their need for comprehensive information [10]. At the end of 1990, the results of the survey had not been announced. In the new 1990 AHA Directory of Health Care Professionals, health sciences library directors are listed along with medical directors, chiefs of staff, nursing administrators, accounting directors, and all of the other hospital administrative and financial services directors [11]. While the recent recognition of hospital libraries and librarians by AHA is essential and most welcome, it is regrettable that AHA has again failed to include the hospital library in its newly updated edition of Hospital Departmental Profiles [12].

The outreach program of the regional medical library (RML) network is aimed at alerting health care professionals, especially in rural and other underserved areas, to the many national information resources that are available to them. Microcomputers, user-friendly software, and improved communication networks provide access to information that was never before possible. Many hospital librarians feel that their participation in such outreach programs will enhance their status as the basic unit in the biomedical communications network. Others fear that they will be replaced by GRATEFUL MED* and Loansome Doc. The RML program has made it possible for the individual in a very small hospital library to lend as well as borrow and to become an integral part of the biomedical communications network. Last October, the National Library of Medicine (NLM) held a meeting on hospital libraries geared primarily for hospital administrators and individuals from other hospital-related associations. Hopefully, NLM will continue to market the hospital library throughout the health care industry. Hospital librarians to whom we have spoken feel that the 1990s will witness a renaissance, a new beginning for the hospital library similar to the early 1970s.

THE FUTURE It is an exciting time to be a hospital librarian! Old methods of information dissemination are being augmented or replaced by streamlined technologies. There will be fewer freestanding hospital libraries as their institutions are acquired by conglomerates. Without question, priority must be given to access, with acquiring of materials being done more and more within a network or consortium arrangement. The cost of books and journals included in this list makes this self-explanatory. The reality of the time mandates that even the unthinkable may evolve into the thinkable: for example, every small hospital will not have its own library but will depend upon network connections, and paying copyright or use fees may be less expensive than purchasing the materials. It is clear that much information is moving from print to electronic form, and hospital librarians must be prepared to participate in developing electronic highways. There really is not such a thing as free information. This is the time for librarians to demand recognition as full-fledged members of the health care industry.

It is a period of opportunity, not only for hospital librarians who are already actively bringing about desired changes in their institutions, but also for those who are yet hoping to accomplish such feats. In the 1990s, there is a great deal more to being a hospital librarian than knowing about books and journals, important as that is. Librarians who have endured endless encounters with their administrators that have resulted in meager or no support for the library collection will be able to discuss information access from a cost-containment perspective of networking and resource sharing. To the hospital administrator such an approach is far more effective than "I need more money for books and journals." In order to participate in this kind of dialog, the hospital librarian must first determine the specific, unique information needs of the library clientele and explore resources, internally and externally, for meeting those needs. Defining information needs as all the books and journals on the Brandon/Hill list that are not in the library has been done all too frequently in the past. Our selected 198

list of books and journals is an information source, not an information need. In today's dynamic health care marketplace, the hospital librarian can gain a competitive advantage only by becoming flexible enough to seize opportunities as they arise. Competition means marketing, a subject that we addressed as a survival factor in the introduction to our 1987 list [13]. At that time, one of the most powerful marketing tools that the hospital librarian has, the telefacsimile machine (FAX), was not in widespread use in most health sciences libraries. Access to a FAX machine is vital for every hospital library, for it provides an on-demand information capability that has never before been possible. It is now routine to provide information within a few minutes or hours that previously would have taken days or weeks to obtain. Being able to supply information when it is needed, even though it is not available on-site, makes the hospital library a real resource center in the eyes of both medical and administrative staff.

In fact, biomedical information center or information resource center or something similar might well replace the library if that word has somewhat of a drab or dreary institutional image. The most successful hospital librarians have learned early on that they must be aware of the information needs of the administration as well as of the medical staff [14-15]. To ensure their viability, hospital librarians must become involved in hospital programs, rather than sitting on the sidelines wanting to be included, but not being willing to struggle with all the politics and red tape to become involved. In the introduction to our 1989 list, we dealt specifically with quality assurance and patient education as entry points for hospital librarians to align themselves with the patient care team [16]. The various aspects of hospital librarianship that we have written about over the years in the introductions to our lists have really been summaries of what we have learned from hospital librarians themselves. parable facility involved primarily in patient care. It has also been used as a guide for a core collection for a consortium or network sharing library resources. As book and journal prices soar, we believe that cooperative resource sharing may become the primary use of this list. While our work is done with the hospital librarian in mind, previous versions of the list have been used extensively by other health sciences librarians, academic librarians, public librarians, physicians, and teaching programs. The choice of books and journals in the 1991 list is the result of our having monitored current medical literature over the past two years as well as our extensive collection development experience in health sciences libraries. The majority of the publications are clinical in content, although we have included clinically relevant basic sciences books. We have omitted basic sciences journals because most hospital libraries cannot justify the extremely high subscription costs for low-use material, and the FAX machine makes the contents of these journals readily accessible. If basic sciences journals are required, they should be purchased on a resource-sharing basis with other libraries. The journal list is not intended to support research activities. We have attempted to assemble a well-balanced current collection of authoritative English language books and journals, primarily in the clinical sciences. Most, but not all, of the publications originate in the United States. When two books appear to be of comparable content and authorship, we have purposely chosen the most recent one. In the 1991 book list, 94.4% of the titles have been published in 1988 or later, and 5.6% between 1983 and 1987. Because we have emphasized current books, some of the standard works that have not been revised for several years have been dropped from this list. Newly updated editions of several standard works that did not appear in the 1989 list have been reinstated in this one. We are aware that "newer" is not necessarily "better." Current literature has been our priority in this endeavor.

The journal list is not intended to support research activities. We have attempted to assemble a wellbalanced current collection of authoritative English language books and journals, primarily in the clinical sciences. If at all possible, we have tried to include inexpensive alternatives to expensive books, but in many subject areas inexpensive quality books no longer exist. In choosing publications for this list, we have taken into account comments, criticisms, and suggestions from health sciences librarians, subject specialists, and Bull Med Libr Assoc 79(2) April 1991

clinicians. The final choices have been our decisions. Price information has been supplied by publishers and dealers and is as accurate as we could obtain at the time of publication. Price increases are to be expected. This list is, as the title implies, limited to print media-books and journals. A hospital librarian who must purchase audiovisuals or microcomputer software, but lacks knowledge of such media, should request assistance from another hospital librarian who has initiated a successful media program or from an academic media center. Approximately $77,700 would be needed to purchase all of the 607 books and 140 journals (1991 subscriptions) on this list-$60,000 for books and $17,700 for subscriptions. Items suggested for first purchase, 297 books and 58 journals, are designated by asterisks, since many hospital librarians cannot afford or may not need all, or even most, of the listed publications. The cost of the asterisked books and journals is about $29,300-$21,700 for books and $7,600 for 1991 subscriptions. Book price totals are based on one-volume editions in instances when books have been published in either one- or more expensive two-volume formats and on "nonmember" prices when indicated for association publications. The W. B. Saunders Company uses a net pricing policy instead of designated list prices for their books distributed by vendors and a single-copy price for books sold directly by Saunders to individuals and libraries. Because of the net pricing policy, prices can vary from vendor to vendor, although prices among dealers for Saunders publications are similar for the most part. Since most hospital libraries purchase books from vendors, we have used a medical book vendor's prices for Saunders books instead of the Saunders'

single-copy prices. The increases in book and journal prices for titles in this list and its previous versions are shown in Table 1. From 1965 to 1991, there has been an increase of 558% ($15.00 to $98.73) in the average price per book title. During the same twenty-six year period, the average price per journal subscription has increased by 810% ($13.90 to $126.52). The book titles in this list are divided into fiftythree subject categories. Forty-five of the categories have average book prices of $50.00 or more; nineteen are $100.00 or more. The category having the highest average price ($184.98) is orthopedics; the lowest, patient education ($36.97). Note again that these average book prices are per title, not per volume. The category of radiology and imaging ($147.91 average price) has a notable effect on the average book price because its thirty-seven titles far exceed the number in other categories. There continues to be an exceptionally large number of radiology books being published that cover both the general subject and every part of the body. Even with thirty-seven titles, we 199

have only sampled the number of new books in radiology and imaging that are available. This category should be viewed selectively since it is unlikely that any hospital would want to purchase all of these books. Without the radiology books, the average book price is $95.53, $3.20 lower than the overall list average of

$98.73. In this list there are only twenty-two nursing books as compared to fifty-five in the 1989 list. Since we have already compiled a list of nursing publications [17], we felt we could better use the space here to include additional books in other disciplines. In previous lists, the nursing books had lowered the average price considerably because of their large numbers, but that is no longer true. Without the nursing books the average price rises to $100.76, a difference of $2.31 from the overall average list price of $98.45. The nursing section contains a number of new books that were published after we compiled the 1990 nursing list. The figures stated here apply only to publications that appear in this list or have appeared over the years in its predecessors. The "per title" calculation includes the cost of all volumes in a multivolume set. The average volume price for the 706 volumes which make up the 607-title book collection is $84.88, $13.85 less than the "per title" average. In instances when a title appears in either one or two volumes, it has been considered as one volume in the physical volume count. Although titles vary from list to list, the cost figures are representative of those being incurred by the small medical library in acquiring current books and journals. Most of the journal titles in the list are domestic, and therefore, the tremendous increases charged by many foreign journal publishers to North American libraries are not reflected. of the book titles and 54% of the journal titles in the 1991 list cost $100.00 or more.

SOURCE LISTS This listt was begun in 1965 because there were few guides available for the novice hospital librarian who needed help in selecting biomedical literature. Other lists came and went, but they were not regularly updated and soon became obsolete. This one has persevered and provided a structured approach to collection development for hospital librarians. Equally as important, it has given hospital librarians a tangible means of enlightening their administrators about the cost of books and journals. Another list, "A Library for Internists VII," updated triennially since 1973, reports the recommendations from the American College of Physicians relating to book selection for the internist and for the hospital library [18]. The MIDCOR List compiled by Karen Hackleman is geared specifically toward librarians in very small institutions and is revised periodically [19]. Our list and the two that have just been mentioned bear the recommendations of their compilers. Medical Books '91 brings together information about books from many health sciences publishers in a convenient form [20] and is especially useful to hospital librarians. ious medical book vendors and is sometimes distributed under variant titles. This publication originates from the medical book trade and is not considered as evaluative in perspective. A number of reference books are included in this list, but we have not tried to present comprehensive coverage of reference sources for the hospital library. Extensive information on biomedical reference materials can be found in Roper and Boorkman's Introduction to Reference Sources in the Health Sciences [21]. However, the last edition of this book was published in 1984 and definitely needs to be revised. The Interagency Council on Library Resources for Nursing compiles "Reference Sources for Nursing" [22], which is updated biennially. This is an up-todate guide that a hospital librarian can use in developing a reference collection because it includes not only nursing reference items but standard biomedical works as well. Each December or January, the American Journal of Nursing publishes "Books of the Year," a subject listing with a brief description of forty or fifty of the year's books selected as "best" by nurses having expertise in the various subject areas [23]. A core collection of books and journals on health care administration compiled by the staff of the AHA Resource Center should be of special interest to hospital librarians [24]. Our list of recommended allied health publications should be useful for hospital librarians who need to develop these special subject areas of their collections [25]. At one time association publications were relatively inexpensive and were purchased heavily by hospital librarians. However, these publications are becoming more expensive each year, and in some instances, their prices are equaling those of commercial publishers. Publication catalogs and brochures from such organizations as the American Dental Association, AHA (American Hospital Publishing), the American Medical Association, the American Nurses' Association, the American Psychiatric Association (American Psychiatric Press), the Catholic Health Association of the United States, and the National League for Nursing are all useful selection tools for the hospital librarian. Organizing and Administering the Small Hospital Library [26] and Manual for Librarians in Small Hospitals [27] are written especially for inexperienced hospital librarians. No hospital librarian should be without Hospital Library Management [28], the authoritative work in the field of hospital librarianship. More theory and advanced detailed information on the various aspects of health sciences librarianship can be found in the Handbook of Medical Library Practice [29]. The Bulletin of the Medical Library Association and the MLA News are essential for professional development and current awareness. care administration, both as a reader and as a contributor. Few librarians are publishing in the literature read by hospital administrators.

PURCHASE MECHANISMS Purchasing books and journals from reliable medical book vendors and subscription agents avoids reams of unnecessary paperwork that occur when these materials are ordered directly from publishers. In addition to providing centralized billing, health sciences book and journal vendors provide important ancillary services to their customers such as newsletters, budgeting assistance, online ordering, electronic billing, and catalog cards. Service is the chief criterion on which both book vendors and subscription agents must be judged by their clients. Institutional purchasing agents routinely see only bottom-line figures when high discounts and low service charges are being offered; overlooking the quality of service can be a costly error both to the library and to the institution. Therefore, the librarian must make the purchasing agent understand that poor service on the part of a book or journal vendor is expensive and is not containing costs when compared to the institutional labor expense incurred to compensate for poor performance on the part of an inept vendor. The most reliable information regarding the quality of a vendor's service can be obtained from local hospital librarians. If librarians expect effective performance on the part of vendors, they should supply accurate, complete order information and should oversee the claiming of missing journal issues in a responsible manner. Claiming the first issue of the year in January when it is published in March is not responsible claiming. Claiming is not one of the strong points of some automated serials systems.

Books from major medical publishers in the United States are generally discounted at 10% by reliable medical book dealers. Discount rates on books from other publishers vary; there may be no discount or at times there may be a service charge. Some scientific and medical publishers do not give discounts on retail orders from libraries, even though their books can 201

Some agents adjust their fees in relation to the title mix (foreign and domestic) and the total dollar amount of the library's account. Others base service fees primarily on the dollar amount. A mediocre subscription agent can be even more devastating to a hospital library than an inefficient book dealer. A low service charge resulting in low performance on the part of the subscription agent can be very expensive. We have not included examination review books in the book list. In teaching hospitals, these publications can be very much in demand in the library. Medical Examination Publishing Company, McGraw-Hill, Appleton & Lange, and Wiley all publish well-known series of review and self-study books 202

in the health sciences that are designed to prepare individuals for state, national, and specialty board examinations. If this type of material is being acquired for the collection, the hospital librarian should contact these publishers and request catalogs. The major book in this field is Rypins' Medical Boards Review, which was updated in 1989 [30]. We have not attempted to cover consumer health publications. If there is a need for this material, The Consumer Health Information Source Book should first be consulted [31]. There are also several other references and lists that deal with consumer health publications, such as a list of popular medical books for public libraries [32], a basic reference list for consumer health information [33], a list of information sources based on patient questions [34], a report on popular health periodicals [35], and a description of a consumer health information library in a hospital setting [36]. In the journal list, we have recommended Index Medicus and Cumulated Index Medicus, but Abridged Index Medicus is adequate for small hospitals that are not engaged in educational and research programs. Abridged Index Medicus can also serve to supplement online acess to MEDLINEt. We have selected twelve of the W. B. Saunders "Clinics." This publication format has been popular among clinicians over the years. The hospital librarian should be aware that there are now forty-four different "Clinics" covering most of the medical specialties from a patient care perspective. This list is used extensively in reprint format, and we do want it to be current as long as possible. Therefore, when we knew of a new edition of a book that was scheduled to be published sometime in 1991, generally that is the edition we have included even though it may not be available at the time this list is published. If a new edition is tentatively planned for 1992 or later, the current edition, if still in print, is listed with a notation regarding the new one. If the previous edition is not in print, then the 1992 one will be noted. This list, as well as our nursing and allied health lists, will be updated between the published biennial revisions in A Major Report [37], a newsletter compiled and edited by the senior author. If a book has more than two authors or editors, the notation "[and others]" is made after the name of the senior author or editor. In the index of authors and editors, we have included only the senior persons; coauthors and coeditors do not appear. When the name of the original author or editor is an integral part of the book title, we have also listed that name in the index. of publishers' names, the imprint on a book may not necessarily be indicative of the publisher presently distributing the book. Therefore, we have tried to cite the books using the names of the publishers who are currently distributing them. As a matter of course, we routinely indicate the publisher from whom a book can be obtained in the United States. In our introduction, we asked fundamental questions about the status and future of hospital libraries in a time of economic austerity and even challenged the relevance of this list for the hospital librarian, whom we asked to embrace change with the same rapidity as telecommunications and computer technology. We firmly believe that the provision of effective information services and the delivery of quality health care are inseparable.
